TRAMADOL + ACETAMINOPHEN (TRAM uh dol)

Ultracet has two generic components - Tramadol and Acetaminophen - and belongs to the opioid analgesics group of drugs. It is akin to a narcotic-like pain reliever; it gives relief from pain and also lessens fever in a patient. Tramadol is a hydrochloride salt, while Acetaminophen [commonly known as Paracetamol] is considered to be the most widely-used analgesic and antipyretic agent. Ultracet is believed to block pain signals in the brain, thereby blunting the pain.

This medicine is used in the treatment of Fibromyalgia, Arthritis [considered to be particularly beneficial in the treatment of Osteoarthritis], and for providing relief from moderate to acute pain post-surgery in dental procedures.

Fibromyalgia is characterized by widespread muscle pain and tenderness; it can occur as a primary condition but patients suffering from Rheumatoid Arthritis, Systemic Lupus Erythematosus, or Ankylosing Spondylitis are also at risk of developing Fibromyalgia.

Arthritis, which is a chronic ailment, affecting joints and muscles, causes inflammation of the affected parts and extreme pain especially in Rheumatoid Arthritis [which affects children also] and Osteoarthritis. Since Tramadol does not produce gastrointestinal side affects that are associated with NSAIDs, it has become a popular medicine for providing relief from pain to arthritic patients.

This medicine has also been found to be successful in giving relief to patients who suffer from pain post-surgery [dental surgery].

The maximum dose recommended is 8 tablets a day, every 4-6 hours, as advised by the doctor/dentist. The dosage differs from patient to patient, depending upon the underlying disorder; it differs from adults to elderly people.

The tablets can be taken with water, with or without food, but you must take your medicine the same way each time. Ultracet works best if it is taken at the onset of pain; if it is taken when the pain has worsened, the desired result may not happen.

Its safety for children below 16 has not been established.

Side effects that commonly result with this medication can include:-

• Nausea, even Vomiting


• Vertigo


• Constipation


• Drowsiness


• Dry mouth


• Miosis [contraction of the pupil]


• Orthostatic hypotension [when the blood pressure comes down when a person stands suddenly].

Stop using the medicine immediately if you experience any allergic reaction to this medicine, which may have the following symptoms:- Rash, Hives, Labored Breathing, or Swelling in the face-lips-tongue-throat.

You should stop taking this drug and contact your doctor immediately if any of the following symptoms, or any other unusual symptoms, occur, while you are taking Ultracet:-

• Bradycardia [slow heart rate]


• Tachycardia [rapid heart rate]


• Ureteric or biliary spasm


• Confusion, Delirium, or Hallucinations


• Muscular rigidity


• Fever


• Hypothermia


• Urinary retention


• Raised intracranial pressure


• Comatose condition.

The drugs that interact with Ultracet include:-

• Any other analgesic containing Acetaminophen like Tylenol.


• MAOI antidepressants like Parnate and Nardil.


• SSRI antidepressants such as Fluoxetine [Prozac].


• Tricyclic antidepressants such as Tofranil and Elavil.


• Anticonvulsants like Carbamazepine [Tegretol].


• Psychoactive & antidepressants like Nefazodone.


• Muscle relaxants like Flexeril.


• Cardiac antidysrhythmics like Lanoxin [Digoxin].


• Antihistamines like Phenergan.


• Narcotic pain relievers like Vicodin and Percodan.


• Sleeping aids like Restoril and Halcion.


• Tranquilizers like Valium and Xanax.


• Anti-coagulants like Warfarin [Coumadin].

If you suspect overdose, immediately contact your emergency room. US residents can call their US national poison hotline at 1-800-222-1222. Symptoms of overdose could include: cardiac arrest, coma, seizures, pallor, profuse sweating, nausea, vomiting, depressed breathing, lethargy, feeling unwell or loss of appetite.

Take the missed dose if there is a lot of time left for the next dose, otherwise skip the missed dose and take the next one. Never overdose.
